The easiest way to apply is:
1. Save the Word file on your computer (PLEASE DON'T OPEN. SAVE IT)
2. Fill in the downloaded Word file
3. Attach the completed Word file and email to Debora Rizzuto
An alternative is:
1. Print out the pdf file
2. Fill in the form with a pen
3. Send it by fax or regular mail
